Bruce's vision was a dream sequence that was caused by a toxic attack by Ivy Pepper.

Description 

The sequence was set in the future where Bruce Wayne is a billionaire after Ivy Pepper applied a small scratch to him that slowly caused toxic plant organisms to develop in him. It begins with Bruce Wayne's face being surgically removed by Ra's al Ghul under the alias Dr. Greenthumb.

During the vision, there is a party at Wayne Manor, with the attendants including Commissioner Jim Gordon (sporting a mustache), his wife Barbara Kean, Oswald Cobblepot (with a tophat), Selina Kyle, Dr. Leslie Thompkins (dressed in an Egyptian god-esque costume), Harvey Bullock and Ivy Pepper.

It also featured a more brutal and angrier Alfred Pennyworth in his soldier's attire who would kidnap Bruce and bring him to the same alley where his parents were murdered. Here Bruce sees a glimpse of his future self.

Bruce wakes up in a cave where he is confronted by a shadowy figure before being swarmed by bats. The vision ends when Jim Gordon and Lucius Fox administer the antidote to Bruce causing him to wake up.

Trivia

• This is the first appearance of Bats in Gotham.
• This vision foreshadowed Ra's al Ghul coming back to life in Season 4.
• This vision served as a way for Bruce to come out of the teenage angst that had overcome his character in the aftermath of killing Ra's.
• Leslie Thompkins' Egyptian-god esque costume could possibly be a reference to the Sphinx of Egypt which was said to tell a riddle to those that wanted an object of desire that the sphinx would guard. This could be a reference to Leslie's relationship with Ed Nygma.
